PROCEDURES FOR ASSIGNING CASE MANAGERS 09/10

 

 

1.    Submit the student roster / list to Special Ed Services support staff.

Include:

·        NAME of the case manager

·        Name of school

·        Name (s)  of other team members providing direct services

    (Speech; OT; PT)

 

2.    The list can be sent via fax, ( 2308), email attachment, or typed in an email.

 

3.    EXCEPTONS:  Speech Only KIDS

·        SLPs will have rights to change case manager, when Speech is the Primary Disability

 

4.    Deadline:  Thursday, August 20th

 

5.    No Case Manager Report will be printed and reviewed at the 1st Contact Teacher mtg on Tuesday, August 25th.
